gather data and the plan sponsor to document
the steps they are taking towards ERISA compliance,
there are 21 worksheets available that
can be printed and used individually, or presented
as a workbook to accompany each education guide
in the process of understanding and documenting
fiduciary compliance.
These worksheets are fully customizable
and printing can be done by designees in-house,
outsourced, or ordered through FSS.
These worksheets include:
1. The Employer Report Card:
This score sheet will give plan sponsors
an idea of how well they are currently fulfilling
their responsibilities and contributing to the
overall success of their plan - and help service
professionals identify shortfalls that can lead
to opportunities to step in and provide a solution.
2. Identifying Service Shortfalls:
This worksheet can help a plan sponsor review
the services on their existing plan and identify
areas of possible improvement - and help service
professionals identify weaknesses of the prospect's
current provider.
3. Goals and Objectives: This
worksheet is a value-added tool to help the
plan sponsor document the goals and objectives
for their plan, and give them a valuable starting
point for benchmarking the success of their
plan against written goals and objectives.
4. ERISA Compliance Checklist:
This worksheet is designed to evaluate the
knowledge level of the plan sponsor and address
any fiduciary issues that could hinder plan
success.
5. The Employee Report Card:
This score sheet is designed to gauge participant
retirement readiness, evaluate how much they
understand about their plan, and is a valuable
benchmarking tool for monitoring the success
of the plan's participant education program.
6. The IPS IQ: This worksheet
is designed to evaluate the knowledge level
of the plan sponsor and educate them on those
issues they may not be aware of.
7. The IPS Development Checklist:
This worksheet allows the plan sponsor to
document their decision making process as it
relates to the management and review of the
investments within their plan, and to assist
in developing a customized Investment Policy
Statement for the plan.
8. SAFE Fiduciary Compliance
Questionnaire: Since a large number of our
students used Fi360 software (including the
instructors), FSS developed a checklist that
could be used with the plan sponsor to document
their answers to the Fi360 SAFE checklist for
Investment Stewards, so that the answers could
then be transferred to the Fi360 website to
utilize their free analysis tool (for which
they did not have an actionable checklist to
gather data).
9. Fi360 IPS Worksheet: Since
a large number of our students used Fiduciary
Analytics software (including the instructors),
FSS developed a checklist that could be used
with the plan sponsor to gather the data
necessary to input into the Fiduciary Analytics
tool, IPS Generator. (When this was developed
Fi360 did not have a printable worksheet for
gathering data for this tool.)
10. Plan Holdings Worksheet:
This worksheet is to document what investments
a plan currently holds so that an investment
review can be conducted.
11. Committee Responsibility
Check-Up: This worksheet helps the plan
sponsor identify areas where they need to implement
a documented process to help show prudence in
the ongoing management of their plan.
12. Vital Stats: This worksheet
is designed to document general information
about a company and their retirement plan.
13. Administrative Options:
This worksheet documents the issues plan sponsors
feel are important to have in their plan from
the administrative side.
14. Plan Expense Worksheet:
This worksheet allows a plan to document
the expenses being paid on their plan to help
them understand total plan expenses, and to
provide data necessary for benchmarking their
plan against industry averages.
15. Education Evaluation: This
worksheet is designed to evaluate a plan's current
education program and identify possible areas
for improvement.
16. Education Program Options:
This worksheet lets plan sponsors review
what options are available on their plan and
to check off which options they would like to
have available to participants when building
a custom participant education program.

This worksheet allows participants to select

for those that choose the premium subscription).

worksheet is to help employees identify their

19. Sponsor Education Needs:
This worksheet allows plan sponsors to document

20. SPD Review Worksheet: This
worksheet guides an investment committee through
a review of their summary plan description to
solidify specific plan basics and start the
fiduciary education process.
21. Planning Calendar Worksheet:
This worksheet is used to create a one-year
planning calendar for the plan sponsor to help
them stay focused on the critical plan management
activities, and keep them updated on current
issues based on methods they find the most effective.
